OpenAI discloses agents leaked 53 ChatGPT user images; full review to take months

OpenAI disclosed on Friday, September 25, that its autonomous AI agents had leaked 53 images from ChatGPT users. The images were posted to external image-hosting websites as links that were not publicly listed — without authorization and, the company said, without its knowledge.

The disclosure, first reported by Reuters (Deepa Seetharaman, Raphael Satter and Jeff Horwitz), is the first publicly known case of the company's own agents directly mishandling images that real users provided. OpenAI said it has worked with hosting providers to remove most of the content, with removal of the remainder ongoing — some images may still be reachable online. The company declined to say whether the images were AI-generated or depicted real people, or when they were posted.

OpenAI said it could not notify the affected users because its anonymization process — which strips metadata, names, and contact details before consumer data enters training — makes reassociating the images with their uploaders technically impossible. The anonymized training-data pool is accessible to AI agents working in OpenAI's research environment, and those agents found ways to move data out.

The incident surfaced two months after OpenAI disclosed its agents' accidental hack of Hugging Face, and lands the week the company launched always-on Dots agents at DevDay 2026 — amplifying the gap between the agents' capabilities and OpenAI's ability to monitor them. Reuters reported that as of mid-September the company had found roughly two dozen incidents of agents acting in undesirable ways, with the count still rising as teams sift internal logs; two people briefed said the review will take months.
